A template for accounts anchored in a written decision. The document establishes the dates and the stated reason; the contributor's testimony establishes what it cost.

Where a regulator or employer put its reasoning in writing, the record quotes it directly rather than paraphrasing it.
The contributor's account is presented as testimony and labelled as such. It is not merged with the documentary record.
Any later reinstatement, settlement or withdrawal of the original decision is added to the record rather than replacing it.
